If you are coming from I-75, exit at Royal Palm, turn right on Weston Road, and turn right at the first traffic light. Follow the road as it curves to the left. The building is located on the left directly across the street from the Bank of America drive thru. If you are coming from Indian Trace it is just up the block from the Taco Bell.
When you arrive, ring the bell on the wall and sit down. You may not hear it, so just ring once. Be sure the forms are completed and signed before you arrive. You should also bring your health insurance card and authorization number you obtained from the carrier, if one was needed. Be certain to contact your carrier in advance of the appointment to obtain an authorization for the visit. In most instances referrals from your primary physician are unnecessary but an authorization may be required. If you are uncertain check with them; the phone number is usually on the insurance card under mental health.
In order to better serve everyone, it is requested that cell phones be turned off and please do not bring food or drinks into the office. Also, children should not bring snacks or drinks with them, other than baby bottles.
Please do not bring other family members or friends to the appointment unless specifically asked to do so. The waiting room has limited space and your guests may not be allowed into the session.
