Payment Options

It is your responsibility to contact your carrier to get an authorization, if one is needed. Please be sure to contact them prior to making an oappointment. Also, while we will normally verify benefits and get co-pay information, if you do call your carrier be sure to ask about required co-payments for behavioral health. The co-pay amount is not always on the insurance ID card, or the amount listed may be incorrect.

Payments accepted include cash, checks and credit cards (Visa, Mastercard, and Discover). Please note that when you receive credit card statements the charges for my services will be listed as coming from ProfCharges.com.

I am on the panels of most HMOs and PPOs including the following

  • Aetna

If you are unsure, call the number on your insurance card and ask if I am on the panel. Most of the time I am a member, even if it’s a carrier I am unfamiliar with.

If you do not have insurance, choose not to use it, or my services are not covered, please call to discuss fees.

If you would like to use a credit card please print and complete the authorization form, and bring it to your appointment.
